Discharge planning
Discharge planning is an integral part of the treatment at Newmarket House. During each stage of treatment patients are encouraged to develop and put into practice the life skills necessary to maintain their recovery after they are discharged from treatment. They learn to handle food and to prepare simple meals for themselves. We encourage exposure to normal social experiences such as occasional meals out, catching a bus into the city, shopping, cinema and theatre trips and, of course, home visits. This offers the opportunity for patients to evaluate, with their key workers, how well they are able to manage such situations and to address any anxieties that may have arisen.
Throughout their admission there is a continuous assessment, in collaboration with the referring team, of the eventual needs of the individual.
Before a patient is discharged a planning meeting is held to evaluate the aftercare resources available locally to ensure that a suitable structure will be in place for them in order to maintain progress.
