Discharge planning and Aftercare
Discharge planning is an integral part of the treatment at Newmarket House. During each stage of treatment there is a continuous assessment, in collaboration with the referring team, of the eventual needs of the individual. It is essential that a support system is in place post discharge, so that they will continue to prosper once they are living independently.
Patients under the age of eighteen cannot access the rehabilitation stage of treatment in Chesham House. These patients, and others for whom treatment in Chesham House is not considered an option, spend the second stage of their treatment in Baskerville House developing and putting into practice the life skills necessary to maintain their recovery after discharge. Patients learn to handle food and to prepare simple meals for themselves. We encourage exposure to normal social experiences such as occasional meals out, catching a bus into the city, shopping, cinema and theatre trips and, of course, home visits. This offers the opportunity for patients to evaluate, with their key workers, how well they are able to manage such situations and to address any anxieties that may have arisen.
For all those leaving residential care, whether from Baskerville House or Chesham House, our Aftercare service is able to provide extensive follow up, tailored to the individual's own needs. This includes support in planning a structure to the day, dietary advice and a helping hand with practicalities. Members of the Aftercare team meet the ‘client', as the individual is now known, at the Clinic, in their own home or in a social setting, to monitor progress and help them manage any anxiety. Clients continue to attend the Clinic to meet with their psychiatrist and their individual therapist. Family therapy also remains available to those who require it. Contact continues, at a decreasing level, until the team is confident that the individual is able to live a fully independent life without further support.
In the case of a client returning to another part of the country, close liaison is maintained with their referring psychiatrist and team. A planning meeting is held to evaluate the aftercare resources available locally to ensure that, after being discharged from Newmarket House, a suitable structure is in place for them.
“It is only now I can see how much I have grown and changed. So many things have been made possible through having the Clinic as a stable platform from which to take that leap of faith into the unknown. There is not a moment where I regret my time at the Clinic.I know that through the support and immense investment in me by such a committed team I am able to go out into the world considerably better equipped to deal with the highs and lows of life” E.T.
